will provide notice that on December 14, 2006, John Sumpter, Vice President Regulatory
of Pac-West Telecomm, Inc. ("Pac-West"), Richard Rindler of Bingham McCutchen,
LLP and the undersigned met with John Hunter, Chief of Staff and Senior Legal Advisor
