Division (SLD) ofthe Universal Service Administrative Company (USAC) has made its

period for appealing this decision to the Federal Communications Commission (FCC). If

? The reco~d shows that your appeal is being made ona Form 471 for which a
Commitment Adjustment Decision Letter (CALywas issued on June 2, 20H). "
